2025年9月18日、米国は国際保健政策の重大な転換点となる「アメリカ・ファースト・グローバル・ヘルス戦略」(AFGHS)を立ち上げました。

構造的な主要変更として、2025年7月に米国国際開発庁(USAID)が解体され、PEPFARを含むその機能が国務省へ移管されました。
A key structural change involved dissolving USAID in July 2025 and transferring its functions, including PEPFAR, to the State Department.
